Liver disease is a significant health concern in China and for foreigners living here. China has the world’s highest burden of hepatitis B — an estimated 87 million chronic carriers — and non-alcoholic fatty liver disease (NAFLD) is rising rapidly with dietary changes. Liver disease screening in China for foreigners is straightforward, inexpensive, and often reveals conditions that have been silently progressing for years.

Why Liver Screening Matters in China

Hepatitis B and C are the leading causes of liver cancer and cirrhosis in China, accounting for over 380,000 cancer-related deaths annually. Less than 25% of chronic hepatitis B carriers in China have been diagnosed. For foreigners, the risks are twofold: potential exposure in a high-prevalence environment, and the dietary and lifestyle changes of expat life that can accelerate fatty liver disease.

Liver Function Panel

The basic panel includes ALT, AST, bilirubin, albumin, and alkaline phosphatase. These enzymes and proteins indicate liver inflammation and function. Cost: ¥50–150. Abnormal results trigger further investigation — a single elevated ALT does not diagnose disease but warrants follow-up.

Hepatitis Screening

Hepatitis B screening uses the five-test panel (HBsAg, HBsAb, HBeAg, HBeAb, HBcAb) which determines infection status, immunity, and disease stage. Hepatitis C screening uses anti-HCV antibody testing. Both are inexpensive — ¥100–300 for the full panel. Vaccination against hepatitis B is recommended for all unvaccinated adults in China, regardless of occupation or lifestyle.

Liver Ultrasound

Abdominal ultrasound detects fatty liver, cirrhosis, liver masses, and biliary abnormalities. It is non-invasive, radiation-free, and costs ¥100–300. Fatty liver is visible on ultrasound before liver enzymes become abnormal, making it an early warning test.

Advanced Assessment

For those with abnormal results or known liver disease, transient elastography (FibroScan) measures liver stiffness — a marker of fibrosis and cirrhosis — without biopsy. This costs ¥200–500 and is available at major hospitals. MRI with elastography provides even more detailed assessment for complex cases.

  • All adults — hepatitis B vaccination status should be confirmed
  • Anyone with abnormal liver enzymes on routine testing
  • Those with risk factors: heavy alcohol use, obesity, diabetes, high cholesterol
  • Healthcare workers and those with potential blood exposure
  • Anyone with a family history of liver disease or liver cancer
  • Long-term residents in China — hepatitis B exposure risk is higher than in many home countries

Practical Considerations

Fasting for 8 hours before liver function testing improves accuracy. If you have had hepatitis B vaccination, bring records — the antibody test will confirm whether immunity persists. If you test positive for hepatitis B or C, China has excellent specialist care and treatment costs that are a fraction of Western prices. Many antiviral medications are available at low cost through the public system.
