Cigna, Allianz, and Bupa Coverage in China: How a Checkup Claim Really Works

Many expats assume that a major international policy means straightforward Cigna, Allianz, or Bupa coverage in China for a health checkup. It often does not work that way, and the surprise usually arrives after the screening, when a claim is partly or fully declined. International private medical insurance is built first around treatment for illness and injury, not preventive wellness, and each insurer treats routine screening differently. Some plans include an annual wellness or health-screen benefit with a fixed cap, others cover only tests triggered by a symptom, and the hospital you choose determines whether you can settle directly or must pay first and claim. This guide explains how coverage actually works, what gets pre-authorised, and how to structure a China checkup so the payment side does not undo the medical plan.

What These Policies Generally Cover — and Exclude

Treatment and diagnostic care versus screening

The core distinction runs between diagnostic and preventive care. A scan or blood test done because you have a symptom, an abnormal prior result, or a doctor’s clinical suspicion usually falls under outpatient or diagnostic cover, subject to deductibles, waiting periods, and policy limits. The same test done as part of an asymptomatic annual package is classified as screening and may sit under a separate wellness benefit, be capped at a modest amount, or be excluded entirely. A coronary calcium scan ordered for chest pain is a different claim from the identical scan bundled into an executive package with no symptom. Frame the indication correctly and have it documented.

How Cigna, Allianz Care, and Bupa Global tend to differ

These brands — Cigna Global/Healthcare, Allianz Care, and Bupa Global — each offer multiple plan tiers, so the brand alone tells you little; the specific plan and its table of benefits decide. Higher-tier plans commonly include some health-screen or wellness allowance, while core or budget tiers focus on inpatient and major outpatient treatment and exclude routine packages. Geographic coverage, the hospital network, direct-billing availability, and whether mainland China is in your rated area all matter. Read your certificate’s wellness, outpatient, China-coverage, and exclusions clauses rather than relying on the insurer’s name or a sales summary.

Direct Billing, Networks, and Pre-Authorisation

Cashless care is hospital-specific, not card-specific

Showing an insurer card at a hospital desk does not create cashless treatment. Direct billing works only where that specific hospital or international department has a provider agreement with that insurer and an active guarantee process. Major Chinese cities have network hospitals for Cigna, Allianz, and Bupa, but the same city also contains excellent hospitals that are out of network, where you pay and claim back. Before booking, verify with both the hospital and the insurer that the department is in-network for your plan and that the planned service is eligible. Confirming this is part of what we do when planning a checkup.

Pre-authorisation and the guarantee letter

For higher-value items such as MRI, CT, endoscopy, or anything approaching admission, insurers usually require pre-authorisation and issue a guarantee of payment (GOP) to the hospital. The hospital or you submit the diagnosis, clinical reason, expected cost, and provider details; approval can take hours to a few working days. Walking in without authorisation can mean paying on the day even for a covered test, or a reduced payout later. Routine screening below a threshold may not need a GOP but still has to fall within your wellness cap. Never assume — get the authorisation in writing before fasting morning arrives.

Why Checkup Claims Get Declined

The common, avoidable reasons

Declines and shortfalls cluster around a few causes. The package contains excluded screening items such as broad tumor-marker panels or full-body scans that no symptom justifies; the hospital is out of network and the reimbursement rate is lower than expected; the deductible or outpatient co-insurance applies; a waiting period for a benefit has not elapsed; or the item was never pre-authorised. Sometimes the claim is coded as “wellness” when it had a genuine diagnostic indication that was simply not written down. A clear doctor’s note linking a test to a symptom or prior abnormal result is often the difference between cover and decline.

Pay-and-claim realities and timing

If you pay up front, gather the itemised invoice, the medical report, the payment receipt, and bank details, and submit through the insurer’s app or portal within the policy deadline, typically within months of treatment. Reimbursement is usually to the policy currency and can take weeks, and it reflects eligible amounts, not necessarily what you paid. Budget for the possibility that the wellness benefit does not cover the full package. Our case studies illustrate how we separate claimable diagnostic tests from discretionary add-ons, and the cost page at China medical cost comparison puts the numbers in context.

Dental, vision, and vaccination limits

Do not assume your medical plan covers everything in a premium checkup brochure. Dental examination or cleaning, detailed optometry, travel vaccinations, and some wellness add-ons frequently sit under separate, lower limits or are outright excluded even on otherwise comprehensive international policies. If a package bundles these, expect to pay for them yourself regardless of how the core blood work and imaging are settled. Knowing this in advance stops a seemingly covered morning from producing a string of small, unexpected charges.

How to Plan a Covered Checkup in China

Sequence the medical and insurance steps

Do the clinical planning first: define which tests fit your age, symptoms, and history rather than buying a fixed executive tier. Then map each item against your plan — diagnostic versus screening, wellness cap remaining, in-network hospital, and authorisation threshold. Choose an in-network international department where direct billing is confirmed for the eligible items, pay out of pocket only for the genuinely discretionary add-ons you choose, and keep clean documentation. If you are entering China specifically for the care, check the entry route on our medical visa checklist.

What we verify before you book

We confirm the exact plan benefits and geographic area, whether each proposed test is diagnostic or screening, the remaining wellness allowance, in-network and direct-bill status at the chosen hospital, and whether a GOP is required and how long it takes. That converts a vague “my insurance covers it” into a concrete plan with a known out-of-pocket amount. Send us your situation and your insurer/plan tier through the contact page, and we will build both the medical schedule and the claim path so coverage works instead of just existing on a card.
