Choosing a city is usually the first decision in a China medical trip, and the guide is that hospitals cluster unevenly. The largest medical systems sit in a handful of cities, each with its own strengths, language support and travel patterns. Beijing, Shanghai, Guangzhou and Shenzhen offer the deepest specialist benches and the most experience with international patients, alongside higher travel costs and longer domestic connections. Second-tier hubs such as Chengdu, Hangzhou, Xi'an, Nanjing, Wuhan and Suzhou often match on clinical capability for specific specialities while being easier to reach from certain regions and cheaper to live in during a stay. Practical selection criteria: the specific department you need rather than the city's general reputation, whether that department runs an international or English-friendly service desk, whether an appointment can be secured within your travel window, and how well the city connects to your home airport including the number of transfers. It is also worth knowing that some care involves more than one trip. A city may be ideal for a screening but impractical for follow-up, so build the itinerary around where the results and the next consultation will actually happen rather than around the cheapest flight. If you need a second opinion, ask whether records can be reviewed remotely first, which often removes the need to travel at that stage.
Base the choice on the department you need rather than on city size or price alone. A second-tier hub with a strong cardiology or ophthalmology department may be the better fit than a larger city with only a general one. City, department and travel logistics should be compared together.
Often, but not always. A screening may be completed at one hospital while follow-up or a procedure goes elsewhere. Confirm the full pathway in advance, including where results are issued, so your travel plan matches the clinical plan.
