Understanding the basics helps you approach them with confidence.
How TCM Herbs Differ from Western Supplements
Unlike Western supplements, which are often single ingredients, TCM herbal formulas combine 5 to 18 ingredients in precise ratios designed to work synergistically. The combination balances the properties of individual herbs, reduces potential side effects, and targets specific TCM patterns of imbalance rather than single symptoms.
These describe the energetic effect of the herb on the body, not its physical temperature. A “warm” herb raises body energy, while a “cold” herb clears heat and inflammation.
How Formulas Are Prescribed
A TCM practitioner does not prescribe a formula based on a Western diagnosis. Instead, they identify your TCM pattern through tongue and pulse diagnosis, then select a formula that matches that pattern. The same Western condition can have different TCM patterns in different people, requiring different formulas.
This is why TCM herbal treatment is individualized. What works for one person with insomnia may not work for another person with insomnia, because their underlying patterns may differ.

Granules and Pills
Dry granules or compressed pills that do not require boiling. They are convenient for busy expats and traveling foreigners. Granules are typically pre-processed and can be dissolved in hot water. Pills are taken like Western medications. These are less potent than decoctions but far more practical.
Proprietary Formulas
Standardized formulas manufactured in pill, capsule, or liquid form. These are widely available at hospitals and pharmacies. They are good for well-defined patterns but less customizable than practitioner-formulated herbal formulas.
Your practitioner will provide specific instructions. Typical regimens include:
- Decoctions: Drink the entire prepared liquid, divided into 2-3 doses per day
- Granules: Dissolve one sachet in hot water and drink, usually twice daily
- Pills: Take 3-6 capsules per dose, 2-3 times daily, after meals
- Blood thinners and herb combinations that increase bleeding risk
- Blood pressure medications and herbs that raise or lower blood pressure
- Blood sugar medications and herbs that affect glucose
- St. John’s Wort and TCM herbs that affect serotonin
- Use prescription formulas from a licensed TCM physician, not random herbs from a market
- Purchase herbs at the hospital pharmacy or a licensed TCM pharmacy with a valid license
- Check the pharmacy’s license number before buying
- Keep the prescription and receipt for your records
Preparing Herbal Decoctions
- Use non-metallic pots for decoctions (glass or ceramic, not iron)
- Follow the practitioner’s instructions for water amount and boiling time precisely
- Strain through a fine mesh cloth or dedicated strainer
- The liquid is typically very bitter. Some patients add a small amount of honey, but ask your practitioner first
Duration of Treatment
- Acute conditions: 2 to 4 weeks of treatment
- Chronic conditions: 4 to 12 weeks, with breaks in between
- Maintenance therapy: seasonal adjustments as recommended by your practitioner
Some licensed TCM pharmacies in China can ship formulas to your home address, but this is subject to both Chinese export regulations and your country’s import rules. Not all formulas can be shipped internationally. Discuss this with your practitioner at the time of prescription.
